Office Policy:

     *Thank you for selecting our office to provide Chiropractic care to you and your family.  Before services can be initiated it is essential you are aware of the billing and payment policies of this office.  Please make note of the following…

     *Payment is expected at the time of service except in situations where prior arrangements have been made.  Forcey Chiropractic Center does not bill your insurance company, you will be responsible for billing your insurance; we will provide you with a detailed receipt to attach to your claim form or we can print out claims on approved medical HCFA 1500 forms.  **There are two exceptions to this; Forcey Chiropractic Center is required by law to bill Medicare and Medicaid (Access) claims.    

     *We have a $46.00 limit on the charges that may be run up on an account.  This is to protect you from an overwhelming balance due on your account.  At such time as your balance reaches $46.00 you must pay your balance off or make arrangements to do so.  You will be required to pay for any additional visits occurring during the payment schedule, you will not be able to charge any amount to your account until such time as the past due balance has been cleared from your account.

     *Forms of payment accepted are cash, check, and VISA/Mastercard.
 
     *We have an answering machine for your convenience.  We will return your call as soon as possible.  Please try to keep your scheduled appointments during our regular office hours, since there is an additional charge for after hours office calls. This fee is not reimbursable by your insurance company. 

     *Our office hours are 8:30am to 12:00pm and 2:00pm to 7:00pm Monday, Wednesday, and Friday and Saturday hours are by appointment.

     *After your initial visit you will be given a treatment plan.  We request that you follow that plan to get the optimum results we both desire.  If you need to change an appointment, please try to come in again as soon as possible so the continuity of your treatment will not be interrupted.  Your progress will be monitored closely with periodic re-examinations.  There may be increased fees for office visits requiring an extraordinary amount of time such as a new injury, re-examination, or additional treatment.

     *If the doctor recommends any supplies or supports, payment is expected at the time of purchase.
